make logging optional. allow to pass configuration constants at compile time.

log_access -> print mime-type

add more mimetypes.

add mimetypes.csv and mime.sh to create list of mimetypes, to be used for mime_map.

remove c99, some methods seem to be unknown by c99 (implicit declaration of function), eg. fdopendir.
