Added to_refactor, containing some RL environments and learners.

Updated documentation, small bugfixes.

Improved documentation. Factored out BaseEnvironment from CartPole. Added FrozenLake environment. Updated learners to be consistent with new structure.

Updated documentation on EM and K-means. Fixed variable name in evolution RL.

K-means and EM updated to work with clusters of different sizes. Data creation functions in utils now also support this. Removed some commented code. Updated readme.
