Update hostname to reflect changes at Instrumental; fix two collector functions that had copy/pasta from increment where the variable names didn't exist

add installation instructions

update version and keywords

fixes from testing. now connects to send metrics instead of keep a persistent connection. removed exception and changed to callback. updated docs

i think it is done. time for some testing
