added clarity script to better track for UX improvement

Fixed the bug which was affecting other exams to be updated in the Questions List page.

Added loading states and visual indicator for users when toggling app settings and added debounce to prevent abuse.

Tried fixing the notification not opening the redirect url and instead showing error
