Fixed undefined reference to max in toGLSLInstruction.c and toGLSLOperand.c

Hull shader input arrays now sized using gl_MaxPatchVertices

Fix issue 33 - wrong dest address for atomics.

Only one join phase was being decoded (issue #32)

Build fix.
