Merge pull request #182 from tom7/release

Use size_t for indices in the routines that merge channels into arrays of RGBA floats. Without this, exr_image.width * exr_image.height (ints) can overflow and cause the floats not to be populated (or worse).

Fix some uncontroversial typos in comments/error messages.
